The trigger to this can be traced back to any Quest where you have to check into a locker to remove your weapons. (for me this was a Songbird Quest; Ive also heard of this trigger being Cloud's Check in locker.)

Dec 22, 2020 · The blue Overwatch rifle is marked with this exclamation mark even to the point that crafting higher levels of the rifle does not consume the blue one. But, these items with the exclamation mark also cannot be removed from your inventory even by putting them in the stash. But they still take weight to carry around. Also, they clutter the inventory.

Loot not going to inventory. Weird new bug since updating to 2.0 and starting a new game. I go to pick up loot, like new weapons or health pickups etc, but it just isn't in my inventory at all.
