I had a look at your survey and have been able to get it working with just a few minor changes. Note that as you have 50,000+ choices in your external choice list, the survey is very slow to load and respond. You only appear to have one filter "level" in your csv: the department field. Each species has a different survey date range. Pronghorn are surveyed between Sept. 1-30. Elk are surveyed between Aug. 1-31 and Deer are surveyed year round.
